#1: Fill Out Your Care Plan
With your Koda account, login and fill out all the parts your care plan.
#2: Sign Your Plan
Make your plan official by signing your documents by December 31st.
#3: Gift a Plan
Once you’ve signed your plan successfully, you can gift a Koda account to someone else.

Share the gift of peace of mind to your spouse, parents, children or even a family friend. Provide the needed information to the form on the right and submit. We’ll send out an email letting them know they can access Koda free of charge and build their own advance care plan.
